Enrollment Requirements
Consultation with the Director and an on-site visit with parent(s) and prospective student is required. Child must be 18 months old (and walking) to 6 years old. The following forms must be completed and turned in prior to enrollment in the program:
- Application Form and $50 non-refundable application fee
- Signed Acknowledgement Form
- Immunization Form - signed by the physician
- Audiogram and/or Speech Report - only required for children with diagnosis or delay
- Copy of child's social security card
- Copy of child's birth certificate
- Copy of child's insurance card
- Copy of both parent's driver's license and any other persons who may pick up the child
Assessment and Trial Period
Upon enrollment, each child will attend Marty's Center for a trial period of 30 days. During the 30-day trial period, the teacher, paraprofessional, and speech pathologist will observe the child during the daily classroom routine. After the child appears comfortable with the classroom setting - then assessments in a natural environment will begin. This comfort level assures that accurate assessment is reflected on the child. The Brigance Inventory of Early Development III Standardized Edition will be utilized to determine the child's current level of performance and shared with the parents to determine if the child is appropriate for the program or if a referral for additional assessment is requested.
Each child with hearing-impairment or speech and language impairment that meets Tennessee or Georgia eligibility requirements will have an M-team meeting and an Individualized Education Plan (IEP) will be designed and written uniquely for that child. An IEP is established on each child based on current assessment and needs.
